Passport Appointment

We are a US Government Passport Acceptance Agency.  We can process the DS-11 form only.  We can process up to 4 family members in one appointment time.

All applicants, regardless of age are required to be present.  If applicants are minors, both parents are required to be present.  
 

With completed DS-11 forms, applicants must present their US Birth Certificate or Naturalization documents and driver's license.  If you have been issued a previous passport, please bring it. 

Please note the US State Dept. will only accept a personal check or money order as payment.  You will need one check or money order per application.  

Library processing of $35 per application and photos can be paid by cash, check or credit/debit card.  If you no longer need your appointment or are no longer available, please cancel your appointment as the demand for passport appointments is great.
