Event Details

We are a US Government Passport Acceptance Agency. We can process the DS-11 form only.  We can process up to two applicants in one appointment slot.

With completed DS-11 forms, applicants must present their US Birth Certificate or Naturalization documents and driver's license.  If you have been issued a previous passport, please bring it. 

Please note the US State Dept. will only accept a personal check or money order as payment. You will need one check or money order per application.  

Library processing of $35 per application and photos can be paid by cash, check or credit/debit card.  If you no longer need your appointment or are no longer available, please cancel your appointment as the demand for passport appointments is great. 

If you are more than 10 minutes late to your appointment, you will need to reschedule.
